I am really interested in what others are doing to look for patient safety events proactively rather than just waiting on someone to report it in. The example I give on this podcast is looking for hypoglycemic events on the medical floors. How many are ok in your system? What else should we be looking for in terms of patient safety? Anyone looking for pseudoaneurysms post cardiac cath by searching the vascular imaging data? What else can we do proactively?
